    Type of Program: Intensive wraparound service for children and youth who are transitioning from a high level RCL 12-14 Group Home Placement to a lower level of care, or at high risk of a RCL 12-14 Group Home Placement
    Description: DSS Wraparound Services provides family-strength based individual, family, group, collateral, case management, and one-to-one services in the home, school, and community, to support the transition from Placement back to the community to those Chapter 26.5 Students identified by the Mental Health Department, Foster Children/Youth identified by Human Services Agency, and Probationers identified by the Probation Department. Services are provided in partnership with FICS-FamilyVision (SB 163) and Human Services Agency.
